IBC Submission Forms

Institutional Biosafety Committee Services (IBCS) Submission Form
The Institutional Biosafety Committee Services division of WIRB provides biological safety review of recombinant DNA used in gene transfer research. Two forms are provided:

Part A initiates establishment of the IBC. It does not have to be resubmitted for additional protocols at the same institution unless the information has changed.

Part B initiates work on protocol review. Each new protocol requires a separate Part B Form.

Use this form for reporting serious adverse event to your IBC.

Institutional Biosafety Committee Services (IBCS) RRI Report Form
Use this form for reporting research-related incident or illness to your IBC.

Web Conferencing for IBC Meetings

New NIH Safety Reporting Option Available
The FDA-NIH Safety Reporting Portal [www.safetyreporting.hhs.gov] unveiled May 24 will accept reports of adverse events in human gene transfer trials and is expected to expand to other clinical trial reporting. Reports from gene transfer researchers do require PI registration with the NIH GeMCRIS system to set up an account [http://oba.od.nih.gov/rdna/oba_gemcris_pi.html]. Reporting on-line does not replace reporting to IBC Services.
